How to fill out the Schengen visa application form

The Schengen visa application form is a standardised document used across all 29 Schengen member states. Here is a section-by-section guide to help you complete it correctly.

Personal information
Enter your surname and first name exactly as they appear in your passport. Include all given names, even ones you don't use daily.

Surname at birth
Your family name at birth, if it later changed (for example, after marriage). Leave it empty if it never changed.

Date and place of birth
Use the format DD/MM/YYYY. Enter your place of birth as it appears in your passport, including the country.

Current nationality
Enter your current nationality. If you hold more than one, use the one matching the passport you'll travel with.

Passport details
Enter your passport number exactly as printed, along with the date of issue, expiry date and issuing country. Make sure your passport is valid for at least 3 months beyond your planned departure from the Schengen area.

Current occupation
Be specific. Instead of just "employee", write your actual job title. Students should write "student" and name their institution.

Purpose of the journey
Select the main purpose of your trip: tourism, business, visiting family or friends, medical, study or other. You can usually select more than one if your trip has several purposes.

Main destination
Enter the Schengen country where you'll spend the most days. This is the country whose embassy normally handles your application.

Member State of first entry
This is the first Schengen country you'll physically enter, which may be different from your main destination.

Number of entries
Single entry means you enter the Schengen area once. Multiple entries means you can enter and leave several times within the visa's validity. Choose based on your travel plans.

Intended dates of travel
Enter your planned arrival and departure dates. These are the dates you'll enter and leave the Schengen area, not the validity period of the visa itself.

Previous Schengen visas and fingerprints
Indicate whether you've held a Schengen visa before and whether your fingerprints have already been collected for a previous application.

Residence and contact details
Provide your home address, email and phone number. If you live in a country other than your country of nationality, you'll also be asked for your residence permit details.

Signature
Once your form is ready, print it, fill in the place and date by hand, and sign it. The form is not valid without a signature. Bring it with you when you submit your application.
