Yes. After downloading, print the form, fill in the place and date, and sign it. Bring it with you when you submit your application.

What if my application is rejected?
The form itself is the official application form. A rejection depends on your personal circumstances and the embassy's decision, not on the form preparation. Visa centres and embassies are also usually understanding about small corrections made to the form on the spot.
